Job Title:Counselor
Home Office:Alexander County Office (ACO) Union County Office Manion (UCOM) or Johnson County Office (JCO). Location is dependent on office availability.
Area of Focus:Behavioral Health
Program(s):PATH- Projects for Assistance in Transition from Homelessness helps adults experiencing homelessnessor marginal housing and those living with significant mental illnesstransition to more stable housing by coordinating services that help them address their individual needs.
Immediate Supervisor:Community Transitions Director - Daniel Hyson
Hours of Work:Full-time 40 hours/week. Participates in after-hours crisis response rotation.
FLSA Classification: Non-exempt
Counselor Minimum Qualifications: Bachelors Degree in Human Services or related field;Qualifies as a Mental Health Professional (MHP).
Counselor Special Requirements:Travels to off-site/community based locations for direct service delivery; participates in after-hours crisis response rotation; maintain and provide proof of a valid drivers license; maintain and provide proof of personal vehicle liability insurance; at least 21 years of age.
Counselor Skills & Knowledge: Knowledge of various behavior and human development theories and practices; local health and human services resources; basic crisis intervention and conflict resolution techniques; excellent written and oral communication skills; excellent computer skills.
Counselor jobduties include but not limited to
  • Carries a caseload as assigned by direct supervisor.

  • Provides outreach and direct service to consumers in the community in their natural environment to reduce barriers to accessing services.

  • Intervenes to change attitudes beliefs and behaviors of consumers to improve individual and family level of functioning.

  • Maintains networking and referral contacts with other human service providers bothinternally and externally while assessing barriers for successful engagement treatment and discharge.

  • CompletesPATHenrollment documentation and records accurately in the electronic health record system andinternaltracking systems to allow immediate supervisor to document appropriately.

  • Provides outreach services to engage homeless individuals to link with local resources for shelter entitlements and meeting of basic needs at a minimum of 4 hoursor more per week.

  • Attends mandatory meetings and relatedPATHconferences or trainings as identified.

  • Provides case management services including but not limited to: advocacy linkages to community resources housing services.

  • Provides crisis intervention to stabilize clients condition by utilizing short term crisis intervention techniques and/or referral to other support services.

Salary Range:$46092 - $62067 annually (pay is based on education level years of experience and credentials)
