$83.970 - $108.360 - $121.360The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade for this position will be dependent on a variety of factors including an applicants years of experience unique skills and abilities education alignment with similar internal candidates marketplace factors other requirements for the position and employer business practices.

To provide the professional clinical and technical knowledge skills and expertise required for the provision of comprehensive pharmaceutical care that supports the mission values and philosophy of Pharmacy Services and Sharp HealthCare.
Required Qualifications
- Doctor of Pharmacy (PharmD) Or
- Bachelors Degree in pharmacy.
- 5 Years experience in the advanced practice area;Or
- 3 Years completion of PGY1 residency and 2 years experience in advanced practice area;Or
- 2 Years completion of PGY1 residency and PGY2 residency.
- AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED
- California Pharmacist License - CA State Board of Pharmacy -REQUIRED
- Pharmacy-Based Immunization Delivery - Various-Employee provides certificate -REQUIRED
Other Qualification Requirements
- Any BPS (Board of Pharmaceutical Specialties) Certification is required within 6 months of hire into this position. Department to track and maintain this requirement - REQUIRED
Advance Practice Pharmacist License-CA Board of Pharmacy is required within 12 months of hire into this position. The department will track and manage this license. - REQUIRED
Essential Functions
- Review of Prescriptions
Reviews all prescriptions for appropriate dose duration frequency dosage form and indication.
Monitors therapy for potential or actual drug interactions (e.g. drug-drug drug-disease drug-allergy etc.)
Assesses patient understanding of medication regimen and provides education tailored to the patient through the following modalities: First dose teaching; Food-drug interaction counseling; Compliance aids.
Performs medication reconciliation. - Clinical and Educational Services
Has met minimum certification/recertification requirements required for hospital CDTM (Collaborative Drug Therapy Management) services offered at entity as outlined in policies and procedures.
Participates in Clinical Services for example CDTM treatment team participation and other services specific to the job.
Demonstrates knowledge of consult services as evidenced by attainment of consult service goals and proper documentation of actions recommendations and patient outcomes on the pharmacy monitoring forms.
Performs by leaving concise objective legible and professional notes in the patients clinical chart or via clinical intervention notes.
Consistently demonstrates ability to complete formal clinical workload assigned during shift as evidenced by completing all consults and advisory services in a timely manner.
Participates in the selection development implementation outcomes analysis reporting and maintenance of selected pharmacotherapy improvement programs including disease state management programs or other cross-continuum pharmacotherapy programs.
Provides and / or coordinates formal needs-based education.
Educates promotes and assists the coordination of staff involvement in clinical responsibilities with formal certification in key activities and skill sets.
May participate in medical staff advisory groups multidisciplinary teams focus groups task forces or other designated activities.
Provide patient education on medication use and disease states.
Collaborate with other members of the health care team and the pharmacy staff to recommend/review current and new agents / protocols (i.e. formulary status usage guidelines and care maps). - Clinical Consult Services
Provides CDTM services in compliance with P and T approved SHC CDTM protocols.
Reviews patient medication therapies for appropriateness of drug therapy therapeutic duplications allergies demographics minimum and maximum dosing guidelines special population requirements medication history non-pharmacologic therapies socioeconomic and other relevant factors.
Clarifies/ verifies and communicates therapeutic issues plan or problems with prescribing doctor/clinic in a timely professional and evidence based manner and document interventions via care plan and progress notes in a formal standardized fashion.
Consistently responds to and documents drug information and other service requests promptly professionally and in accordance with SHCs Behavioral Standards.
Leads P-Ts identification of medication usage evaluations (MUE) opportunities MUE performance and subsequent performance improvements.
Consistently participates in financial pillar/cost savings measures (e.g. formulary adherence readmission reductions etc.).
Consistently collaborates with all relevant clinicians and non-clinicians with a team player attitude.
Guides the participation of other non-APC pharmacy members involved in CDTM services. - Communication and Teamwork
Customer Service/Interdepartmental Relations (with external customers outside of the pharmacy department):
* Answers phone in timely friendly manner stating department name and title
* Fosters positive relationships through consistent respectful professional interactions with customers and other staff members
* Responds to requests in timely caring manner that is consistent with Sharp Everyday Actions (i.e. Mutal Respect Care and Compassion)
Operational Services (working with internal customers with pharmacy department):
* Clearly communicates goals and instructions to other staff members
* Provides relevant handoffs between shifts (written/oral)
Effectively intercepts and troubleshoots problems as demonstrated by effective problem solving prioritization of issues with respect to level of significance and impact to patient care safety workflow and communication.
When faced with complex situations has knowledge of and applies the appropriate policies and procedures including but not limited to Chain of Command escalation.
Effectively evaluates ongoing workflow issues and directs daily workload to achieve efficiency and maximize resource utilization as business needs require.
Attends and participates in staff meetings or reviews staff meeting communications.
Reads work related email at a minimum on assigned workdays responding in a professional and reasonable timeframe. - Pharmacy Operations
Counsels patients on medication use consistent with professional standards.
Assures controlled substances are dispensed with accuracy. Maintains integrity of all record keeping. Assists with narcotic inventories as required.
Dispenses medication according to policies and procedures and confirms all financial information prior to dispensing (active eligibility etc).
Enters data correctly into computerized patient medication profile and verifies that all technician orders are correctly entered into computer.
Demonstrates proficiency with and utilization of automation and advances in technology.
Works as Clinical advance practice RPh duties.
Participates in cost management initiatives to include assisting patients and physicians identify the most cost effective treatment plan available. - Regulatory Compliance
Monitor compliance with state and federal laws and regulations related to the practice of pharmacy as well as standards and regulations for other regulatory agencies for specialty pharmacy practice.
Ensure documentation meets regulatory audit and compliance.
Ensure protocols are followed.
Ensure collaborative agreement followed. - Safe Medication Practice
Knowledge of and active participation in concurrent medication safety event (MSE) monitoring programs as demonstrated by interventions and RL solution submissions.
Qualitative work demonstrates thoughtful and thorough identification evaluation and analysis of medication safety events. Performs initial analysis of MSEs and assists in the research and identification causative factors of events.
Performs non-voluntary monitoring (i.e. trigger reports harm monitors alerting orders). - Training and Mentoring
Active in personal continuing education and professional development.
Orients and trains new pharmacy staff as assigned.
Provides direction and precepts pharmacist residents interns and students as assigned.
Knowledge Skills and Abilities
- Computer skills are required especially in the application of word processing spreadsheet (including basic analytical functions such as pivot tables) and presentations.
- Expertise with database creation and management is preferred.
