As a Clinician you will coordinate and complete client assessments working collaboratively with the child family legal guardian law guardian psychiatrist nurse psychologist educators child care staff and other providers to prepare implement and monitor a comprehensive permanency-focused case plan for children in the Residential Program. You will report to the Director of the Residential Treatment Center. You will work onsite and in the community.

Responsibilities:

• Assess children and Families at intake to complete a comprehensive mental health assessment to create a diagnosis and comprehensive treatment plan for recommended service needs.
• Meet and communicate with biological families guardians foster families and case workers.
• Provide therapeutic services to identified children/Families based on treatment plan recommendations.
• Attend community-based meetings or sessions when needed.
• Work with an interdisciplinary team to monitor ongoing progress of children and families and update treatment plans.
• Work with referring and collateral agencies and prepare reports and correspondence.
• Maintain Clinical case records following Agency OMH and other state mandates.
• Diligently plan for discharge/permanency at the time of admission.
• Communicate with your client and treatment team

Requirements and Education:

• Requires a Masters degree in Social Work or Related mental health field
• RTF Clinician requires two years of related experience.
• Professional licensure required Limited Permits accepted (LMSW LCSW LMHC LMFT LCAT)
• Valid NYS drivers license required
• Must pass physical.
• Must complete TCI training.
