POSITION SUMMARY

Assists the Director of MDS in the development completion and transmission of the resident assessment in accordance with the state requirements and policies and goals of the facility.

DUTIES AND RESPONSIBILITIES

• Demonstrates commitment to companys mission values and standards of ethical behavior.

• Ensures that all residents are treated fairly with kindness dignity and respect and their rights are protected at all times
• Respects and upholds all residents personal and property rights whenever carrying out duties including knocking before entering a residents room.
• Complies with all company and departmental policies and procedures.
• Participates in development and completion of preliminary and comprehensive resident assessment (MDS) and Care Plan in accordance with current rules regulations and guidelines including the implementation of CAAs and Triggers and Plan of Care.
• Ensures that care plan includes measurable objectives and timetables to meet residents medical nursing mental and psychosocial needs that appropriate health professionals are involved in assessment and that schedule of activities is developed as required.

KNOWLEDGE SKILLS ABILITIES & QUALIFICATIONS

• 1-3 years experience providing direct care to multiple patients/residents one years previous MDS experience in Skilled Nursing environment preferred
• Nursing degree from accredited college or graduation from approved LVN program required RN preferred
• Must possess current unencumbered license to practice as an LVN or RN in California.
• Current valid CPR certification
• Knowledge of clinical standards of practice regulations and reimbursement governing long term care and Medicare participation
