Position Overview
Navitas Healthcare LLC is seeking an experienced and strategic Director of Case Management to lead a dynamic department within a 165 bed acute care hospital. This leadership role is responsible for overseeing all case management operations ensuring effective care coordination optimal resource utilization and alignment with organizational goals.
This position partners closely with executive leadership including the Senior Leadership Team Chief Medical Officer and Medical Director to drive both clinical and financial performance.
Key Responsibilities
Provide overall leadership and direction for Case Management operations including oversight of 10 FTEs and 7 PRN staff
Ensure effective discharge planning processes focusing on timeliness quality and compliance
Collaborate with executive and clinical leaders to achieve departmental and organizational objectives
Monitor and analyze performance metrics to identify opportunities for process improvement
Optimize staffing models competencies and productivity standards
Foster a collaborative inclusive and high-performing team environment
Support staff in resolving complex cases and barriers to care coordination
Qualifications
Education: Bachelors degree required; Masters degree (MSN MHA MBA or related field) preferred
Licensure: Active RN LCSW or LMSW license required
Experience:
Minimum 3 years of acute care hospital case management experience
Minimum 2 years in a leadership role within case management
Core Skills:
Strong leadership and decision-making abilities
Executive presence and communication skills
Ability to drive performance outcomes and lead change initiatives
