Registered Nurse Home Care Case Manager

Position Summary:

The Home Care registered nurse assesses and evaluates patient care needs provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice.

The nurse is responsible to improve or maintain patients health status provide terminal illness care and to prepare patients and their families for self-directed and/or assisted care in their community setting.

The nurse coordinates medical social and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission discharge billing referrals bereavement and care delivery.

Work Schedule: M-F occasional evenings and on-call. Every 6-7^th weekend. 1-2 holidays/year.

Territory:

West (ex: Minnetonka Orono Vicotria Chaska)

Required Qualifications:

• Education Experience or Equivalent Combination:
  • Work experience as a RN for at least two years
• Licensure/ Registration/ Certification:
  • Current MN Registered Nursing License
• Knowledge Skills and Abilities:
  • Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.
  • Proven proficiency in the operation of the following: Personal Computer Telephone Fax Machine Photocopier Printer and electronic medical record.

Preferred Qualifications:

• Experience in home care and or hospice desirable.
• Must be able to function independently with minimal supervision.
• High level of sensitivity assessment skills technical-skills and communications skills.

Demonstrates initiative judgment critical thinking and the ability to problem solve.
