Position Overview:

Greenlife Healthcare Staffing is seeking a dedicated Licensed Practical Nurse (LPN) to join a skilled care facility in Queens NY. This role involves providing compassionate high-quality care to long-term care residents ensuring adherence to care plans and collaborating with interdisciplinary teams to enhance patient outcomes.

Qualifications:

• Education: Completion of an accredited Licensed Practical Nursing (LPN) program.
• Licensure/Certifications:
  • Active New York State LPN license.
  • Proof of Hepatitis B immunity (required).
• Experience:Must have at least one (1) year of long-term care experience in a nursing home setting or one (1) year of hospital experience.
• Soft Skills: Strong communication adaptability to rotating shifts and teamwork.

Key Responsibilities:

• Administer medications and treatments as prescribed.
• Monitor and document patients vital signs and health status.
• Collaborate with RNs physicians and therapists to implement care plans.
• Assist residents with activities of daily living (ADLs) and mobility.
• Ensure compliance with infection control and facility protocols.
• Respond to emergencies and provide timely interventions.
• Maintain accurate and timely medical records.
