About the role

Fixed term full time role until June 2027 for an experienced and skilled Clinical Nurse Specialist (Grade 2). Currently based at St Leonards you will be a part of a highly dedicated Community Palliative Care team that provides the evidence-based nursing care and advice to patients with palliative care needs.

Key Responsibilities

The Care Navigator plays a key role in supporting patients families and health professionals by providing timely clinical triage palliative care advice and care coordination across settings. This role focuses on assessing referrals identifying urgency and needs navigating patients to appropriate services and ensuring continuity of care.

Working as part of a multidisciplinary specialist palliative care team the Care Navigator provides personcentred assessments (in the community and via telehealth) supports symptom management within scope facilitates advance care planning and escalates complex issues to senior clinicians when required. The role involves close collaboration with GPs hospitals RACFs NSW Ambulance and community services to reduce avoidable emergency presentations and hospital admissions.

This position also contributes to service quality clinical documentation and ongoing development of the Palliative Care Navigator Service while acting as a clinical role model aligned with HammondCare values.

About you

  • Current registration as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Agency
  • Relevant post registration qualifications and at least three years experience working in palliative care
  • Current NSW drivers licence (Class C or above)
  • Evidence of integration of theoretical knowledge and clinical skills critical thinking and analysis in your daily practice as a registered nurse
  • Able to work independently and within a multidisciplinary team at a senior level
  • Evidence of strong written and verbal communication interpersonal skills and ability to identify key messages issues and concerns when communicating with others
  • Commitment to ongoing professional development
  • Computer literacy with email and an electronic patient record system
  • Effective time management skills and ability to prioritise competing workloads within a complex environment
