Position Summary
Reporting to the Vice President Strategy Quality Risk & Communications the Director Quality Patient Safety & Experience provides strategic and operational leadership to advance a culture of quality safety and patient-centred care across the Brant Community Healthcare System. The Director is accountable for the design implementation and sustainment of integrated quality management and patient safety programs that support exceptional patient experiences and align with organizational priorities and regulatory requirements.
The Director leads the development of the annual Quality Improvement Plan oversees Accreditation processes and ensures robust patient safety structures including incident management Quality of Care Reviews and safety culture initiatives are effectively implemented. Through strong partnerships with leaders physicians staff patients and families the Director builds organizational capacity in quality improvement promotes a just culture environment and enables evidence-informed decision-making.
Primary Responsibilities
- Leads corporate quality management structures within BCHS including Quality Planning Quality Assurance and Quality Improvement.
- Oversees the organization-wide Accreditation process with Accreditation Canada including all activities required before during after and between surveys.
- Leads the development and execution of the organizations annual Quality Improvement Plan (QIP) and maintains a comprehensive integrated Quality and Patient Safety Program ensuring hospital-wide stakeholder engagement and alignment with strategic priorities.
- Oversees the successful implementation of corporate quality projects including progress reporting stakeholder engagement and risk escalation as appropriate.
- Supports a culture of continuous improvement through the implementation of leading-practice evidence-based quality improvement frameworks across the organization.
- Leads the Patient Safety Incident Management process including Patient Safety Rounds Urgent Patient Safety Touchpoints Quality of Care Reviews
- Participates in the development implementation and achievement of the organizations patient safety goals.
- Provides direction and oversight for all components of the patient safety continuum including fostering and promoting a just culture environment.
- Collaborates with multiple internal stakeholders to facilitate manage and oversee the response to and delegation of patient safety concerns.
- Oversee the Patient Relations process working collaboratively with stakeholders to standardize workflows identify patient concerns/complaints and recommend appropriate resolutions.
- Ensure patient experience surveys are effectively implemented across the organization with timely reporting of unit-level results and support for teams in reviewing data and developing targeted strategies to improve patient experience.
- Oversees the Emergency & Disaster Management Program supporting the development and maintenance of emergency code responses and business continuity plans aligned to leading emergency preparedness practices.
- Collaborates with multiple internal stakeholders to facilitate manage and oversee the response to and delegation of patient safety concerns.
- Oversees the Incident Management (RL6/SIMS) database and reporting system to support quality improvement activities.
- Responsible for ensuring daily operations and departmental objectives are met by coordinating tasks processes and providing clear direction on workflow execution.
Mandatory Qualifications
- Masters Degree in Business Administration Health Administration Quality Improvement Patient Safety or a related field.
- Bachelors Degree in a Regulated Healthcare Program Business Health Administration or a related field.
- Minimum 5 years of progressive leadership experience in quality patient safety and/or patient experience within a healthcare environment.
- Excellent understanding of operational excellence quality improvement change management and service innovation with demonstrated expertise in quality improvement methodologies (e.g. IHI Lean Six Sigma) and design thinking.
- Proven experience facilitating collaborating and negotiating with multiple stakeholders and service providers.
- Regulated Health Professional in good standing with the applicable regulatory college preferred.
- Strong organizational time management and interpersonal skills
- Ability to work independently and collaboratively across all levels of the organization
