Program Overview Cummer Housing Program Supportive Housing Community Care Unit

The Supportive Housing Program provides housing for seniors who have experienced homelessness and may be living with mental health and/or substance use challenges. The program offers onsite psychosocial and primary care supports aimed at promoting stability wellness and self-sufficiency within a supportive community environment.

Residents receive a wide range of supports including intensive case management harm reduction and addiction support therapeutic and supportive counselling 24-hour staffing assistance with activities of daily living nursing services life skills development and social recreational and volunteer opportunities. The programs approach is guided by WoodGreens commitment to community development anti-discrimination and harm reduction principles while working within relevant legislation and regulatory requirements.

Reporting to the Manager and/or Supervisor Supportive Housing Programs the Overnight Support Worker plays a key role in ensuring the safety and well-being of residents and the building during overnight hours. Responsibilities include providing reception and security support monitoring common areas responding to emergencies and resident alarms implementing crisis and de-escalation strategies providing after-hours supportive counselling enforcing building policies and completing required clerical shift exchange and risk management duties as part of a collaborative care team.

What You Will Do
  • Provides reception services to staff and residents living in supportive housing program
  • Ensures the safety of the building and its occupants after regular business hours.
  • Ensures the zero-guest policy is always enforced.
  • Monitors activities in the common areas and the outside perimeter of the building through the security cameras/visual inspections and responds appropriately.
  • Implements crisis response strategies including de-escalation support debriefing etc. when necessary.
  • Completes hourly rounds on the participant floors
  • Provides after hours supportive counselling to residents when required.
  • Re-directs any visitors and responds to issues appropriately.
  • Performs lock up and alarm duties in accordance to established procedures.
  • Responds to the 24-hour resident personal alarm system according to appropriate procedures. Understands how to operate trouble shoot and contact the 24-hour response team for the emergency response system as necessary.
  • Carries out appropriate First Aid responses when necessary.
  • Works effectively with other team members including on site Home Support Workers to address resident and building issues.
  • Completes a detailed participant activity log & shift exchange reporting any incidents occurrences health and safety risks status of resident community resident interpersonal conflict.
  • Understands risk management procedures and responds professionally and appropriately. This includes but is not limited to responding to all emergency situations such as managing a resident emergency (e.g. illness fire in the unit or building) or emergency that affects building operations (e.g. elevator breakdown power failure).
  • Be prepared to work in environments where they might be pest control and hoarding issues
  • Be prepared to work in environments where there may be death and dying
  • Be prepared to work in environments where there may be drug use
  • Other duties assigned by the management team
What You Bring to the Team
  • Two-year diploma in social services or certification in security
  • Minimum three (3) years of demonstrated experience working with older adults and/or seniors with mental health addictions issues and street involvement.
  • Ability to interact effectively with high risk marginalized and vulnerable individuals.
  • Demonstrated ability and experience dealing with aggressive behaviours and effective de-escalation and boundary setting skills.
