Overview
Lifesharing Specialist
Pay: Negotiable based on experience
Status: Full-time 37.5 hours per week
Schedule: Monday - Friday 8:00 am 4:00 pm or 9:00 am - 5:00 pm evenings and weekends occasionally required
Location: Camp Hill PA
What were looking for:
- 18 years of age
- A valid drivers license with at least 6 months of driving experience
- Access to a vehicle with valid insurance registration and inspection
- High School Diploma or GED required
- Associates degree or technical certification within a related area preferred
- 25 years of related experience required; experience in human services strongly preferred
- Ability to communicate read write and interpret English
- Strong organizational planning and communication skills with attention to detail
- Ability to build relationships work collaboratively and maintain confidentiality
- Preferred knowledge of individuals with intellectual and developmental disabilities autism and/or the aging population
- Preferred experience with Medicaid-funded services and PA regulations (Chapters 6100/6500)
- Preferred familiarity with human service program operations compliance and documentation systems
- Preferred background in psychology education social work healthcare administration or related field
As a Specialist Lifesharing Services youll support UCP Central PAs mission by coordinating and overseeing lifesharing placements that promote independence safety and meaningful community living. Youll partner with individuals families and caregivers to develop and monitor support plans ensure regulatory compliance and provide ongoing training and guidance helping create stable supportive home environments for the people we serve
What Youll Do:
- Evaluate referrals and coordinate lifesharing placements ensuring a strong match between individuals and caregivers
- Complete home studies and review caregiver applications to ensure safe compliant and supportive living environments
- Develop and monitor Individual Support Plans (ISPs) collaborating with families and team members to meet individual needs
- Conduct regular home visits and ongoing oversight to ensure quality care compliance and positive outcomes
- Train and support caregivers on person-centered practices safety and regulatory requirements
- Maintain accurate documentation and complete assessments reports and audits within required timeframes
- Collaborate with families Supports Coordinators and external partners to coordinate services and support individuals in the community
