Manager Care Management

Full-Time Leadership Role

Athena Health Care is seeking an experienced Manager of Care Management to lead and oversee all functions within the Care Management Department. This role is responsible for the strategic and operational management of care coordination case management discharge planning utilization management social work services and pre-admission case management.

The Manager serves as a key liaison among patients families clinical teams physicians and external partners ensuring high-quality patient-centered care while maintaining operational efficiency and financial integrity.

• Oversee daily operations and staff within the Care Management Department
• Lead care coordination case management discharge planning and utilization management functions
• Ensure delivery of high-quality patient-centered care aligned with best practices and clinical standards
• Serve as a liaison between patients families physicians healthcare teams and community resources
• Collaborate with managed care organizations skilled nursing facilities and external agencies
• Develop implement and monitor policies procedures and performance improvement initiatives
• Ensure compliance with regulatory and accreditation standards
• Support financial performance through effective resource and utilization management
• Recruit mentor and evaluate departmental staff fostering a culture of collaboration and accountability

Required:

• Masters Degree in Social Work (MSW) from an accredited program
• Active Washington Clinical Independent Social Worker License (LICSW)
• Minimum 6 years of experience in case management
• Minimum 2 years of leadership experience at the Manager or Director level

Preferred:

• Leadership experience in case management within a large hospital setting
• Strong knowledge of healthcare regulations utilization management and discharge planning processes
