Family Medicine Physician

Community Health Services is seeking a compassionate and dedicated Family Medicine Physician to join our collaborative team-based primary care environment. This role offers the opportunity to provide comprehensive patient-centered care across the lifespan while making a meaningful impact in an underserved community.

• Provide preventive acute and chronic care for patients of all ages
• Manage chronic conditions and support long-term health outcomes
• Participate in population health and care coordination initiatives
• Collaborate with a multidisciplinary care team
• Maintain accurate and timely clinical documentation in EHR systems
• Deliver culturally competent patient-centered care

• MD or DO from an accredited medical school
• Board Certified or Board Eligible in Family Medicine
• Active Connecticut Physician License (or eligibility)
• DEA registration and state controlled substance registration (or eligibility)
• Strong commitment to quality efficiency and compassionate care
• Excellent communication and teamwork skills
• Proficiency with electronic health records (EHR)
• Sound clinical judgment and evidence-based approach to care
• Ability to pass background check and drug screening

• 3 years experience in ambulatory or urgent care settings
• Experience working with underserved or diverse populations
• Bilingual (Spanish/English) candidates strongly preferred
