Rates for submission(strictly follow)

Malpractice Must be included in rates.

Orientation $295-307.50- half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

Hourly $Per hour not OT

Holiday $885-922.50 - Must be 1.5x the hourly rate. Per holiday hour no OT

Schedule/Availability Requirements:

8a-5pm - Monday Tuesday & Thursday - Site is flexible on the 3 days please list the 3 days your provider can cover

12-15 shifts per month

Facility Location: St. Anne Hematology/Oncology Clinic - The clinic is attached to the hospital via the skybridge.

Duties: Managing exisiting hematology and oncology patients for a physician who is

Required Procedures: No procedure.

EMR: EPIC Beacon

Patients per day: 12-15

Support Staff: RN and an MA.

Reason for Coverage: Backfill current locum

Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less: NO

Air Travel: Local no flights- Preferred- site will consider flights

Credentialing Information:

Credentialing with other facilities No

• Board Certification BC required
• State License WA license or IMLC highly preferred
• Other Certifications BLS required
• Malpractice Clean highly preferred
• Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - REQUIRED
