Job Summary:
We are seeking a dedicated Transplant Registered Nurse to support outpatient transplant services. This role focuses on triage care coordination patient education and clinic support in collaboration with physicians and advanced practice providers. The RN will play a key role in ensuring continuity of care for transplant patients.
Key Responsibilities:
Triage Support:
Manage and triage patient phone calls
Communicate patient concerns effectively to providers (NP/MD)
Prioritize patient needs and ensure timely follow-up
Laboratory Review & Follow-Up:
Collaborate with providers to review lab results
Communicate medication changes and care plan updates to patients and caregivers
Ensure appropriate follow-up actions are completed
Clinic Support:
Assist during outpatient clinic visits in coordination with care teams
Perform medication reconciliation and review of systems
Coordinate follow-ups including labs and medication adjustments
Communicate care plans clearly to patients and families
Patient & Family Education:
Provide education on treatment plans medications and post-transplant care
Support patients and caregivers with ongoing clinical guidance
Complete required forms (e.g. school forms dental clearance)
Assist with prior authorizations
Support additional program-related administrative tasks
Required Qualifications:
Active Registered Nurse (RN) license (state-specific)
Experience in transplant outpatient or specialty care nursing
Strong clinical assessment and communication skills
Ability to work collaboratively in a multidisciplinary team
