Job Summary
Navitas Healthcare LLC is seeking a dedicated Registered Nurse (RN) Transplant to support outpatient transplant services and ensure continuity of care. The RN will play a key role in triage lab follow-ups clinic coordination and patient education while collaborating closely with the care team to deliver high-quality patient outcomes.
Key Responsibilities
Provide triage support by managing patient calls and communicating needs to the care team
Review outpatient lab results and coordinate timely follow-ups
Communicate medication changes and care plan updates to patients and caregivers
Support clinic visits in coordination with providers
Perform medication reconciliation and review of systems
Coordinate follow-up care including lab monitoring and treatment adjustments
Educate patients and families on ongoing care plans and transplant protocols
Assist with administrative duties such as documentation prior authorizations and fax management
Complete required forms (e.g. school and dental clearance letters)
Maintain accurate and timely patient records
Ensure compliance with safety confidentiality and organizational protocols
Qualifications
Graduate of an accredited School of Nursing
Active Registered Nurse (RN) license New York (required)
Minimum 2 years of RN experience (transplant experience preferred)
Strong clinical communication and coordination skills
Ability to thrive in a fast-paced outpatient setting
