Title: Registered Nurse Transplant Location: New York NY Duration: 13 Weeks
Job Overview
This position supports a transplant program during a temporary leave of absence working closely with the clinical team to coordinate patient care across the transplant lifecycle. Primary transplant coordinator responsibilities will remain with internal staff.
Key Responsibilities
Clinical & Patient Care
Assist with pre- and post-transplant patient care
Perform medication reconciliation and review of systems
Monitor patient progress and identify complications (e.g. infection rejection)
Coordinate care before during and after transplantation
Support transplant processes including organ offers recipient preparation and evaluations
Triage & Lab Coordination
Triage patient calls and communicate needs to NP/MD
Review outpatient lab results with providers
Communicate medication changes and updates to care plans
Clinic Support
Assist during clinic visits in collaboration with physicians and nurse practitioners
Coordinate follow-ups lab work and medication adjustments
Communicate updates to patients and caregivers
Patient & Family Education
Educate patients and families on:
Medication adherence
Post-operative care
Long-term transplant health maintenance
Administrative Duties
Manage documentation including faxes and patient records
Complete school forms and clearance letters
Process prior authorizations
Support general transplant program operations
Required Qualifications
Minimum 1 year of RN experience
Active New York RN License (must be submitted with application)
BLS Certification (AHA required)
Bachelor of Science in Nursing (BSN) required
Preferred Qualifications
PALS Certification
Experience with EPIC charting system
Pediatric transplant experience
Core Skills
Pre- and post-transplant care (heart kidney liver lung)
Transplant evaluation and patient assessment
Medication management and contraindication review
Recognition and management of transplant-related complications
Knowledge of transplant waitlist criteria and regulatory guidelines
Strong patient education and care coordination skills
