Client Care Coordinator The Palms (FlexHome Program)

Schedule:MondayFriday 7:00 AM 3:00 PM

We are seeking a dedicated and dynamicClient Care Coordinatorto lead daily operations within our FlexHome program at The Palms. This role is ideal for someone who thrives in a fast-paced environment enjoys building relationships and is passionate about delivering exceptional care while growing a program.

What Youll Do:

  • Manage daily scheduling to ensure seamless care delivery for all clients
  • Serve as the primary point of contact for clients families and community partners
  • Provide hands-on care support when needed to maintain service excellence
  • Oversee program operations and ensure high-quality standards are consistently met
  • Train and support caregivers ensuring proper skill alignment and client matching
  • Actively engage with residents and prospects to grow program census
  • Monitor performance metrics and drive results aligned with key performance indicators

What Were Looking For:

  • Strong organizational and scheduling experience
  • A relationship-driven professional with excellent communication skills
  • Experience in home care senior living or healthcare preferred
  • Ability to lead train and motivate caregivers
  • A proactive problem-solver who takes ownership and drives outcomes
  • Passion for improving the lives of seniors and supporting their independence

Compensation & Benefits:
