Job Summary
We are seeking a dynamic and experienced Director of Case Management to lead and oversee all aspects of case management operations within an acute care setting. This role is responsible for driving care coordination discharge planning and effective resource utilization while ensuring compliance with regulatory standards and improving patient outcomes.
The ideal candidate is a strong collaborative leader with a proven track record in acute care who can effectively manage interdisciplinary teams and foster a high-performing patient-centered environment.
Responsibilities
- Oversee daily operations of the Case Management department
- Lead and optimize care coordination and discharge planning processes
- Ensure compliance with hospital policies regulatory standards and accreditation requirements
- Manage staffing productivity and departmental performance
- Collaborate with executive leadership (CFO CMO CNO) and key stakeholders
- Analyze operational and clinical data to identify and implement process improvements
- Drive team engagement accountability and a positive work culture
- Support resolution of complex cases and ensure continuity of care
Required Qualifications
Education:
- Bachelors Degree (required)
- Masters Degree in Nursing Healthcare Administration or MBA (preferred)
Licensure:
- One of the following active licenses required:
- Registered Nurse (RN)
- Licensed Clinical Social Worker (LCSW)
- Licensed Master Social Worker (LMSW)
Experience:
- Minimum 3 years of acute care hospital case management experience
- Minimum 2 years in a leadership role within case management
