Key job responsibilities
- Support a broad range of outreach for patients supporting the Medicare Population Health Programs team (this includes but is not limited to outreach for quality population health screenings and more)
- Conduct targeted outreach to patients to close quality gaps for multiple measures as informed by the quality strategy and market performance with specific targets
- Implement targeted Medication Adherence programs for at-risk patients for multiple markets including chart review and direct outreach
- Consistently meet or exceed key performance indicator (KPI) goals
- Supports Medicare Population Health Program Needs including chart reviews and data entry for Population Health Screening Programs
- Manual gap closure through portals and application of annotations suspects or subscribed marker sets to support population health initiatives.
- Chart reviews as directed to support our population health strategy
- Experience with data analysis and basic Excel skills
- Experience completing complex tasks quickly with little to no guidance and react with appropriate urgency to situations that require a quick turnaround or experience in a fast-paced high-tech company
- Experience communicating and delivering presentations to customers stakeholders and/or teammates
- Experience working independently and collaborating effectively in a remote team environment
- 1 year of experience working as a Health Coach Lab Service Specialist or Member Support Specialist is required
- Knowledge of Medicare quality measures
- Interest in learning how to interpret clinical data and its connection to day to day care.
- Experience with generating reports in MS Excel or Google Sheets

The starting pay for this position is listed below. Final starting pay will be based on factors including experience qualifications and location.
