The role
Here at Each we are looking for passionate Care Coordinators to join our care finder program supporting older people who may be experiencing vulnerability isolation or difficulty navigating the aged care system.
This role is all about meeting people where they are building trust and ensuring they dont fall through the gaps when accessing vital supports.
As a Care Coordinator you will use assertive outreach and a personcentred approach to help older Australians understand access and navigate aged care and other community services. Your day may include:
- Engaging and building rapport with potential clients their families and community intermediaries.
- Helping people interact with My Aged Care including guiding them through eligibility assessments and next steps.
- Supporting clients to identify and connect with the right aged care and community supports through warm referrals and ongoing followup.
- Establishing strong relationships with local government community and voluntary services to promote the care finder program.
- Maintaining accurate case notes and participating in reporting reflective practice and ongoing professional development.
Youll be part of a supportive and collaborative team working both independently in the community and with colleagues across Each.
Who we are looking for
Youre someone who thrives on supporting others and can build genuine connections with older people who may face complex barriers. Youll bring:
- Experience working with older adults or vulnerable community groups.
- Knowledge of local aged care and community supports or the ability to quickly acquire it.
- Strong communication and rapportbuilding skills with empathy and active listening.
- A commitment to personcentred practice respect cultural safety and traumainformed care.
- Solid problemsolving skills and confidence with administrative and dataentry tasks.
- A second language is highly desirable.
Qualifications :
- Qualification in Social Work Human Services Aged Care Community Services Health or similar.
- Current Australian drivers licence and access to an insured vehicle.
