About the job
The full-time Homeless Outreach Specialist at the Hampton-Newport News Community Services Board plays a pivotal role in supporting individuals living with a mental illness or co-occurring substance use disorder who are homeless. This position involves direct outreach to assess and address individuals psychological housing and social needs. The Outreach Specialist will initiate housing plans link clients to service providers and assist in identifying essential resources such as shelter food and medical services. Responsibilities include conducting assessments developing treatment plans and advocating for client needs within and outside the system. The specialist will monitor client progress facilitate appointments and provide supportive counseling to both clients and their families ensuring that all individuals receive comprehensive care tailored to their unique circumstances.
By participating in client review meetings and collaborating with other professionals the Outreach Specialist actively contributes to creating a supportive environment and promoting successful outcomes for vulnerable populations.
requirements
To excel as a full-time Homeless Outreach Specialist at the Hampton-Newport News Community Services Board possessing a thorough knowledge of interviewing techniques supportive counseling principles and social casework practices is essential. Familiarity with community resources local state and federal regulations governing client rights along with the unique needs of the homeless mentally ill and substance-abusing populations is crucial. Successful candidates will demonstrate the ability to evaluate behavioral observations effectively and establish rapport with clients and their families.
To qualify for this position candidates must have the following:
- Bachelors Degree in Human Services and one (1) year of experience working with the target population or Five (5) years of equivalent work experience
- Valid Drivers License
Additionally strong communication skills are necessary for preparing documentation and maintaining narrative and statistical records. The ability to collaborate with other professionals in various public and private agencies is vital for ensuring comprehensive care and service delivery.
