Job Description:
We are seeking a skilled Licensed Master Social Worker (LMSW) to support inpatient care teams with psychosocial assessments discharge planning and coordination of post-acute services. The ideal candidate will have strong hospital-based experience and the ability to manage a high-volume caseload.
Key Responsibilities:
- Conduct comprehensive psychosocial assessments for patients
- Develop and implement discharge plans for inpatient populations
- Coordinate post-discharge care referrals (e.g. home care rehab hospice)
- Manage a caseload of 2530 patients
- Collaborate with interdisciplinary teams including physicians nurses and case managers
- Document patient interactions and plans in EMR systems (Epic preferred)
- Support patients and families through complex care transitions
Key Requirements:
- Masters Degree in Social Work (MSW) Required
- Active LMSW License (New York State) Required
- Strong knowledge of psychosocial assessment and discharge planning
- Excellent communication and interpersonal skills
Preferred Qualifications:
- Inpatient hospital discharge planning experience (strongly preferred)
- Experience with:
- Hospice care
- Hemodialysis patients
- Substance abuse cases
- Guardianship and ethical cases
- Experience with Epic or Allscripts EMR systems
