Home Health RN Responsibilities Include:

  • Perform comprehensive patient assessments and develop individualized plans of care

  • Manage a designated patient caseload and coordinate care across the interdisciplinary team

  • Evaluate patients physical emotional social and environmental needs

  • Conduct initial assessments routine visits and ongoing re-evaluations

  • Monitor patient progress and identify changes in condition

  • Collaborate with physicians therapists caregivers and other healthcare professionals to ensure quality outcomes

  • Educate patients and families on disease management medications treatments and self-care strategies

  • Update and revise care plans based on patient needs and clinical findings

  • Maintain accurate and timely documentation in accordance with regulatory and agency standards

  • Promote patient safety and help prevent avoidable hospitalizations and emergency room visits

  • Protect patient confidentiality and uphold HIPAA standards at all times

What Youll Bring

  • Current unrestricted RN license in the state of practice

  • Minimum of one (1) year of nursing experience in a clinical care setting

  • Strong assessment critical thinking and communication skills

  • Ability to work independently while collaborating effectively with a multidisciplinary team

  • Reliable transportation valid drivers license and current auto insurance

  • Ability and willingness to travel throughout the assigned service area

  • Ability to sit stand bend lift and move throughout the workday including lifting up to 50-100 lbs. when necessary

Qualifications:

  • Associate or bachelors degree in nursing

  • Home Health or Hospice experience preferred

  • 1 Year Clinical RN experience required

  • Experience with HomeCare HomeBase (HCHB)
