Job Summary
Job Title: Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) Long Term Care
Location: Utica NY 13502
Duration: 13 Weeks
Schedule Details
- Shift Type: Day Shift
- Shift Hours: 6:30 AM 2:30 PM
- Hours per Week: 37.5 (guaranteed)
- Days per Week: Varies (includes every other weekend)
Position Summary
The LPN/LVN Long Term Care provides skilled nursing care to residents in a long-term care setting. This role involves caring for patients with complex medical and cognitive conditions including orthopaedic needs multiple sclerosis (MS) dementia and memory care. The nurse is responsible for delivering care in accordance with established protocols maintaining accurate documentation and collaborating with interdisciplinary teams to ensure quality patient outcomes.
Key Responsibilities
- Administer medications and treatments as prescribed; monitor and document patient responses
- Provide care for residents with complex medical and cognitive conditions
- Assist residents with activities of daily living (ADLs) including bathing feeding mobility and hygiene
- Float between units within the facility as required
- Maintain accurate and up-to-date documentation using EMR systems (PointClickCare preferred)
- Follow infection control safety protocols and facility policies
- Communicate effectively with residents families and healthcare team members
- Work scheduled shifts including every other weekend
- Participate in team meetings training and ongoing professional development
Required Qualifications
- Active New York LPN/LVN license
- Minimum 1 year of recent experience in long-term care or a similar setting
- Current Basic Life Support (BLS) certification
- Experience with EMR systems (PointClickCare preferred)
- Ability to float across multiple units within the facility
- Strong communication organizational and patient care skills
Work Environment
- Long-term care facility (approximately 40-bed unit)
- Exposure to residents with dementia memory care needs orthopaedic conditions and multiple sclerosis
- Fast-paced team-oriented environment requiring flexibility and adaptability
