About the role
Purpose of the Job
- To provide a high standard of social work to users arising out of the Councils duties and powers under legislation and in accordance with divisional and area policy procedures guidelines and the Code of Practice for Social Care Workers.
- Operating within a multidisciplinary environment to provide appropriate professional Social Work support with a focus on meeting the social care outcomes as articulated in the Department of Healths Adult Social Care Outcomes Framework.
- A focus on undertaking reviews of care and support needs. This is part of a wider programme of how we commission support particularly from supported living providers.
- Within the context of an enablement approach and operating within an integrated service to provide individualised outcomes-led personalisedsupport based on structured individual assessments that inform the setting up and commissioning of appropriate enablement packages of support.
About you
Registered as Social Worker with the Social Work England.
Training in relevant legislation appropriate to client group
Relevant experience in operating effectively as a practising Social Worker.
Understanding and experience in operating with a comprehensive assessment framework including self-assessment.
Experience in preparing clear and accurate correspondence reports and other written documentation.
