Job Summary

3 Month Contract With A Local Authority

About the Role

The Personal Advisor plays a key role in supporting young people leaving care by providing tailored advice guidance and practical assistance. You will empower young people to actively engage in their pathway planning ensuring plans accurately reflect their strengths aspirations and individual needs.

Working collaboratively with Children Looked After Social Workers and partner agencies you will carry out assessments and contribute to coordinated support that promotes positive outcomes and independence.

This is a dynamic role involving a varied and evolving workload requiring initiative resilience and a strong commitment to delivering high-quality support.




Key Responsibilities

  • Provide advice guidance and ongoing support to young people leaving care
  • Support and facilitate young peoples active participation in pathway planning
  • Ensure pathway plans are person-centred and outcome-focused
  • Undertake assessments of needs in partnership with social workers and relevant agencies
  • Work collaboratively with multi-agency professionals to coordinate effective support
  • Maintain accurate records and documentation in line with statutory requirements
  • Respond flexibly to a wide range of tasks in a fast-paced environment
  • Promote independence wellbeing and positive life outcomes for young people



Requirements

  • Experience working with young people particularly those leaving care or in vulnerable situations
  • Knowledge of assessment processes person-centred planning and care management
  • Ability to work effectively with multi-agency partners (e.g. social workers housing education health services)
  • Strong communication and interpersonal skills with the ability to build trusting relationships
  • Ability to manage a varied workload and work under pressure in a busy environment
  • Good organisational and record-keeping skills
  • A proactive flexible and solution-focused approach
  • Commitment to safeguarding and promoting the welfare of young people
