Job Summary
The Registered Nurse (RN) Long Term Care is responsible for delivering high-quality nursing care to residents in a skilled nursing environment. This role includes clinical care care planning documentation and collaboration with interdisciplinary teams to ensure optimal patient outcomes.
Key Responsibilities
Patient Care
- Administer medications and treatments as prescribed within scope of practice
- Perform initial nursing assessments and assist with MDS and care plan development
- Monitor patient conditions and respond to changes appropriately
- Assist residents with Activities of Daily Living (ADLs) as needed
Clinical Operations
- Maintain accurate clinical documentation of assessments treatments and patient progress
- Participate in shift reports and ensure effective communication of patient information
- Follow all federal state and facility regulations and nursing standards
Team Collaboration
- Communicate effectively with residents families and interdisciplinary care teams
- Provide guidance and limited supervision to support staff
- Participate in staff training and in-service programs
Process Improvement
- Utilize continuous improvement methodologies for workflow efficiency
- Apply problem-solving techniques to enhance patient care delivery
Required Qualifications
- Active Registered Nurse (RN) License
- Knowledge of geriatric/long-term care nursing practices
- Strong communication and interpersonal skills
- Ability to assess analyze and respond to patient care needs
Preferred Qualifications
- Open to recent graduates or candidates with less than 1 year of experience
- Experience in Skilled Nursing Facility (SNF) or Long-Term Care (LTC) settings
