Job Summary
The Case Manager II isresponsible for maintaining a caseload of patients completing all oversight related to patient care by other nursesand coordination with the Director of Nursing to ensure compliance and standards of care are met.
We are looking for individuals that are dedicated and dependable. The Case Manager II must have the following:
- Must hold a current license as a Registered Nurse from the Virginia Board of Nursing.
- Must have experience working in a home care agency.
- Must have direct patient care experience.
Job Duties:
Clinical supervision and oversight visits in the field
Retrieval and review of clinical documentation/progress notes
Monitoring compliance overseeing care plans and assisting the Director of Nursing with regulatory and quality assurance requirements
Provide nurses with education and guidance on site in the homes
- Ability to travel each day and work a schedule to address caseload needs
