Job Title: Registered Nurse (RN) Skilled Nursing / Long-Term Care

Job Summary
The Registered Nurse (RN) provides comprehensive nursing care to residents in a skilled nursing/long-term care setting. This role involves administering treatments coordinating care maintaining documentation and supporting interdisciplinary collaboration to ensure high-quality patient outcomes.
Key Responsibilities
Patient Care & Clinical Duties
- Administer medications and perform treatments as prescribed within scope of practice
- Conduct initial nursing assessments for newly admitted residents
- Assist in the development and updates of care plans including Minimum Data Set (MDS) assessments and quarterly reviews
- Monitor and assess residents medical physical and psychological conditions
- Assist residents with Activities of Daily Living (ADLs) such as bathing dressing eating toileting hygiene and mobility
Communication & Coordination
- Participate in shift reports and ensure accurate information exchange
- Communicate effectively with residents families and interdisciplinary teams
- Ensure continuity and quality of care through collaboration
Documentation & Compliance
- Maintain accurate clinical and administrative documentation
- Ensure compliance with applicable healthcare regulations and nursing standards
- Follow proper protocols for medication management and patient care
Leadership & Support
- Provide guidance and limited supervision to subordinate staff
- Assist team members with direct patient care as needed
- Participate in staff training in-service education and development programs
Quality Improvement & Operations
- Utilize process improvement methodologies and tools to enhance care delivery and efficiency
- Support continuous improvement initiatives and workflow optimization
- Perform additional duties as assigned
Required Knowledge & Skills
- Geriatric/gerontological nursing principles and practices
- Patient assessment monitoring and care planning
- Pharmacology and medication administration
- Knowledge of healthcare regulations and compliance standards
- Strong communication and interpersonal skills
- Ability to assess patient progress and respond to changes in condition
- Problem-solving and critical thinking skills
Qualifications
- Active Registered Nurse (RN) license (state-specific)
- Minimum 1 year of experience in a skilled nursing or long-term care setting preferred
- Ability to work collaboratively in a team-based environment
