Job Summary

The Health and Welfare Council of Long Island (HWCLI) is a private not for profit health and human services planning research/public education and advocacy organization that serves as the umbrella for public and non-profit agencies serving Long Islands poor and vulnerable individuals and families.

The Health Equity Alliance of Long Island (HEALI) is Long Islands Social Care Network (SCN) of community-based organizations (CBOs) and healthcare providers building a person centric model that integrates healthcare social care and behavioral health care. HWCLI is the lead entity for the HEALI Social Care Network. The SCN brings together social service and health care providers from across Nassau and Suffolk counties to provide enhanced healthcare equity through improvements in need identification care coordination integration and provision of tailored funding.

JOB ANNOUNCEMENT: Food is Medicine Director Health Equity Alliance of Long Island

HWCLI/HEALI seeks an experienced energetic and equity-driven leader to build implement and manage HEALIs Food is Medicine (FIM) programa portfolio of food-based nutrition interventions delivered through nonprofit and for-profit providers across the HEALI network and supported by Community Health Workers in coordination with healthcare partners. This role provides strategic leadership technical expertise training and quality oversight to ensure high-impact culturally responsive interventions that support disease management prevention and optimal health by connecting participants to nutritious foods. Develop and operationalize Food is Medicine program that leverages community developed Food is Medicine interventions that may include medically tailored meals clinically appropriate meals nutrition counseling and food boxes.

Responsibilities include:

Strategy & Program Leadership

  • Design lead and continuously improve HEALIs FIM strategy and program portfolio ensuring alignment with HEALI priorities and the evolving Food is Medicine landscape.
  • Develop and operationalize Food is Medicine interventions which may include medically tailored meals medically supportive groceries produce prescription programs and nutrition education.
  • Establish program standards workflows and implementation guidance to ensure consistent high-quality delivery across providers.

Network & Partnership Development

  • Forge and sustain partnerships with community-based organizations healthcare providers and national stakeholders.
  • Curate and mobilize HEALIs provider network to inform strategy and accelerate impact in Food is Medicine and nutrition.
  • Manage complex multi-partner initiatives and multiple workstreams to achieve measurable outcomes.

Clinical Integration & Technical Assistance

  • Collaborate with healthcare partners including primary care providers to integrate Food is Medicine services into care coordination pathways.
  • Provide training and technical assistance to network providers and Community Health Workers.

Quality Compliance & Performance Measurement

  • Serve as subject matter expert overseeing compliance quality assurance and performance of Food is Medicine interventions.
  • Develop and oversee quality improvement and outcome measurement processes including equity-focused metrics.

External Representation & Thought Leadership

  • Represent HEALI as a recognized leader in Food is Medicine and food policy initiatives.

Qualifications

  • Must be a Registered Dietitian through the Commission on Dietetic Registration (CDR)
  • Requires NYS Certified Dietitian-Nutritionist (CDN) certification (or eligible to apply)
  • ServSafe certification (or will obtain within first 30 days of hire)

Preferred Qualifications

  • Five or more years of experience in nutrition public health healthcare or social services programs.
  • Experience with Food is Medicine Medicaid-funded initiatives or healthcarecommunity partnerships.
  • Experience managing provider networks or multi-site programs.

Knowledge Skills and Abilities

  • Strong computer/technology skills include electronic health record Microsoft Office-Suite Outlook and menu analysis software.
  • Strong organizational and time management skills for coordinating and completing various monthly tasks.
  • Strong writing and public speaking skills
  • Ability to relate effectively to diverse groups of people from all social and economic segments of the community with a track record of building authentic constructive relationships with others.
  • Team player who enjoys working both collaboratively and independently.
  • Provides support for assigned Committees.
  • Communicates in a positive manner both internally and externally to partners.
