Job Title: Health Counselor Role Overview

We are seeking a compassionate and proactive Health Counselor to join our this role you will act as a guide for clients looking to improve their physical and mental well-being. You wont just be giving advice; youll be helping individuals navigate chronic conditions lifestyle transitions and emotional hurdles to achieve sustainable health outcomes.

Key Responsibilities

Individual Assessment: Conduct comprehensive health screenings and behavioral assessments to identify a clients physical and emotional needs.
Goal Setting: Collaborate with clients to develop personalized Wellness Action Plans focusing on nutrition exercise stress management and sleep hygiene.
Chronic Disease Support: Provide counseling for patients managing long-term conditions like diabetes hypertension or obesity.
Behavioral Coaching: Use techniques such as Motivational Interviewing to help clients overcome resistance to change.
Case Management: Maintain detailed confidential electronic health records (EHR) and track patient progress over time.
Crisis Intervention: Identify signs of mental health distress and provide immediate support or appropriate referrals to specialists.

Required Skills & Qualifications

Education	Bachelors or Masters degree in Counseling Psychology Public Health or Nutrition.
Certification	Relevant certification (e.g. CHES NBC-HWC or state-specific counseling licensure).
