Details of Service:

The HSE has created six new health regions. Each region is responsible for providing both hospital and community care for the people in that area. Bringing community health services and hospitals together means we can take a more patient-centred approach to healthcare.

HSE Dublin North East provides health and social care to North Dublin Louth Meath Monaghan and most areas of Cavan.

Community Healthcare Services are delivered through the HSE and its funded agencies to people in local communities as close as possible to peoples homes. Community Healthcare Services focus on keeping you well so that you can continue to live at home or close to home through our health promotion disease screening diagnosis treatment and rehabilitation programs. You can refer yourself to most Community Healthcare Services or through your GP public health nurse community mental health team etc. Services include - Primary Care Social Care - Older Persons Social Care Disabilities Mental Health Health and Wellbeing Quality Safety and Service Improvement.

Established to deliver on the HSEs commitment to promoting the protection welfare and safeguarding of adults at risk of abuse. The current team operates under the scope of the HSE 2014 Safeguarding Vulnerable Persons at Risk of Abuse Policy. The client group for this this service is a person over the age 18 with a diagnosed disability (physical intellectual sensory) or any person over the age of 65. Future reform of adult safeguarding policy and procedures are envisaged with the implementation of the Moving Forward Adult Safeguarding in the Health Services Executive Report (2024) in addition to the DoHs The National Policy Framework for Adult Safeguarding in the Health and Social Care Sector in December 2025. The Office of the Chief Social worker HSE has the responsibility to lead on the implementation of this reform programme.

The Team comprises of a PSW 6 SWTL 5 PQSWs and 1 Social Work Apprentice and the region covered is that of the 2 IHAs Dublin North East.
