PACE a member of Empath Healthis seeking a meticulous and goal-driven Enrollment Specialistto join our interdisciplinary PACE team in Tampa FL. In this role the PACE Enrollment Specialist supports the full enrollment cycle for the Program of AllâInclusive Care for the Elderly (PACE). This role engages prospective participants families caregivers and referral sources to explain program benefits determine eligibility coordinate assessments and ensure a smooth transition into services.

The Program of All-Inclusive Care for the Elderly (PACE) is a unique care model designed to help older adults remain safe and independent in their own homes.

- Team-Driven Preventive Focus: Collaborate closely with physicians social workers therapists and other clinicians in a proactive care model focused on prevention wellness and holistic well-being.
What Youll Do
- Respond to inbound calls conduct followâup outreach and convert inquiries into home visits or center tours.
- Run Medicaid eligibility checks and coordinate with the Medicaid team for application support when needed.
- Conduct initial screenings to assess eligibility based on age residency care needs and insurance.
- Schedule and coordinate home visits tours intake appointments and transportation as needed.
- Guide applicants through required documentation including consents insurance forms and state eligibility processes.
- Maintain accurate records in CRM/enrollment systems in compliance with federal and state PACE regulations.
- Collaborate with Outreach Marketing and interdisciplinary teams to support enrollment goals.
- Build relationships with referral partners such as hospitals SNFs community agencies and physicians.
- Participate in community events to promote PACE services.

What Youll Need
- Associate or Bachelors degree in social work human services healthcare administration gerontology or related field.
- Experience working with older adults longâterm care programs or healthcare enrollment preferred.
- Strong communication and interpersonal skills with the ability to explain complex information clearly.
- Problemâsolving and critical thinking
- Knowledge of Medicaid Medicare and longâterm care systems is highly beneficial.
- Ability to work collaboratively with interdisciplinary teams.
- Strong organizational skills and attention to detail.
- Bilingual a plus
