Job Summary

The Dover-Sherborn Public Schools seeks a 1.0FTE Transition Specialist and Out-of-District (OOD) Coordinator to support students ages 1422 in preparing for post-secondary success while coordinating services for students in out-of-district placements (Pre-K22). This position is part of the Dover-Sherborn Educators Agreement. This role provides transition instruction related to employment college vocational training and independent living and collaborates with students families staff and community partners to ensure coordinated individualized transition planning aligned with students IEP goals. The position also serves as the districts LEA designee and primary liaison for students referred to or placed in Massachusetts-approved special education schools collaboratives and other specialized Specialist ResponsibilitiesDesign and deliver transition-focused instruction targeting employability self-determination and independent living students in identifying strengths interests preferences and needs related to their post-secondary formal and informal transition assessments to inform and guide IEP data and monitor student progress toward transition-related benchmarks and life with guidance counselors special educators and vocational staff to align transition programming with academic and career student participation in work-based learning programs internships and job shadowing individualized coaching for community-based work experiences and job small groups of students to work sites community programs or transition activities using a district-approved vehicle (requires a 7D license).Serve as the primary liaison with adult service agencies local employers and community referrals to adult agencies (e.g. Chapter 688) and assist families in navigating post-secondary all IEP transition components meet DESE requirements and are updated in appropriate IEP meetings departmental staff meetings and professional development focused on transition -of-District Coordinator ResponsibilitiesSchedule all meetings including: annual reviews progress meetings and triennial meetings within the appropriate updates regarding referrals IAES placements and extended evaluations. Works collaboratively with Special Education Coordinators when a student is referred for a placement. Work collaboratively with agencies such as DCF DMH DDS DPH and the MRC to address student student attendance and communicate concerns with out-of-district schools and up-to-date transcripts and credit information for high school students by providing documents to the Dover-Sherborn High School school counselors. Manage/share information related to OOD budget with the Director of Student ServicesManage school wide calendars and transportation with the Special Education student files in accordance with DESE district is committed to strengthening opportunities for diversity equity and inclusion.
