DaVita is seeking a Clinical Coordinator to lead dialysis care in a skilled nursing this role youll oversee patient care delivery support a clinical team and ensure treatment meets the highest standards of safety and quality.
Key Responsibilities:
Coordinate care for ESRD patients including labs vils and outcomes
Lead and support a team of clinicians and PCTs
Deliver direct patient care in a skilled nursing facility
Monitor patient status and adjust care as needed
Ensure compliance with all clinical and regulatory standards

Exposure to complex care needs and the opportunity to apply broad nursing skills
Flexibility to float to other clinics and adjust to varied schedules
Qualifications:
RN license in the state of practice
CPR certification
At least 18 months of RN experience
Minimum 6 months of dialysis experience
Charge RN readiness approval required
CNN or CDN certification preferred
ICU CCU ER or Med/Surg experience preferred
Supervisory experience preferred
Proficiency in MS Word and Outlook
