Job DescriptionThe Transition Program Special Educator provides specially designed instruction and case management for students ages 1822 receiving special education services focused on post-secondary outcomes including employment independent living community participation and functional ExperienceExperience working with students with moderate to significant disabilitiesExperience implementing and managing IEPsExperience with transition-age instruction (vocational functional community-based)Experience with functional academics and life-skills instructionExperience with behavior supports and data collectionCore ResponsibilitiesServe as IEP case manager for assigned transition-age studentsDevelop and implement instruction aligned to post-secondary transition goalsDeliver specially designed instruction in:Employment and vocational skillsIndependent living skillsFunctional literacy and mathSelf-advocacy and self-determinationCoordinate and supervise community-based instruction (CBI) and job site learningCollaborate with job coaches related service providers and agency partnersConduct transition assessments and progress monitoringMaintain service logs progress reports and compliance documentationFacilitate student and family participation in transition planningRequired CredentialsValid State certification in Special Education (secondary or PK12 as applicable)Current teaching license in good standingSafety Care/crisis intervention certification Background check and fingerprint clearance
