JOB SUMMARY:
Works as an agency hired Broker for individuals seeking Self-direction services throughout Oswego County. The Broker is to assist the individuals they support to gain access to services and programs and to coordinate the use of those services and programs. The broker also provides person centered services on-going support advocacy and regular follow-up to ensure the persons maximum benefit from services and programs. Brokers must work within all OPWDD program regulations and Agency/Program policies and procedures.
JOB DUTIES AND RESPONSIBILITIES:
Broker Duties:
- Acts as Broker under Self Direction (SD) and meets with individuals family members and outside agency representatives to determine desired supports.
- Facilitates person centered planning sessions assists the individual in building a circle of support and completes mapping as part of developing a SD plan.
- Facilitates discussion on risks and safeguards.
- Supports individuals and families/circle of support to understand responsibilities of receiving SD services.
- Encourages participant to assume as much responsibility as possible for his/her services supports in the process.
- Works with the individual circle of support and the care manager to help identify and develop initial connections in the community that will support the plan/budget.
- Facilitates changes to the SD plan and budget as requested.
- Assists the individual if requested with recruiting monitoring and hiring self-hired staff.
- On a monthly basis monitor detailed expenditures to ensure that expenditures do not exceed the budget.
- Completes the SD budget for approval from the FI and DDRO.
- Attends the launch meeting with the individual and the circle of support to initiate the SD plan; facilitates a minimum of two circle of support meetings per year per individual.
- Responsible for the case files of multiple individuals.
- Respects the individuals right to privacy and confidentiality by appropriate use of written and verbal communication.
- Completes Community Habilitation Plans for individuals who choose to utilize Self-Hired Community Habilitation services.
- Maintains effective working relationships with service providers FI DDRO Liaison community resources and families.
- Ensures compliance with regulations agency policies and procedures.
- Ensures all required documentation is complete accurate and legible; assists participants with submitting documents timely.
- Utilizes EVERO software; assists participants with software use as needed.
- Ensures the agencys incident management process is followed.
- Provides guidance on self-direction principals and ensures compliance with OPWDD regulations.
JOB REQUIREMENTS:
- Must exhibit professionalism sound judgement and excellent verbal and written communication.
- Must have the ability to work with others in a warm non-judgmental manner and act as a positive role model to consumers and staff; must possess good leadership qualities.
- Must be self-motivated with the ability to work independently.
- Must have the ability to facilitate promote and support informed choice and self-advocacy.
- Must have the ability to act accordingly in crisis situations and to resolve conflicts.
- Proficient in computer skills such as Excel Word and PowerPoint. EVERO Software
- Ability to understand and manipulate budgets.
- Must maintain all required certifications and trainings which includes attending the initial Self-Direction Broker Institute training and annual broker authorization requiring 12 hours of training per year.
- Must possess a valid NYS Drivers License with good driving record within Agency policy and have access to a reliable and insured vehicle.
- Must have acceptable physical ability to carry out the position.
- Must possess a valid NYS license with good driving record in accordance with Agency policy and procedure and have access to a reliable insured vehicle; must be able to maintain certification as an Agency driver.
MINIMUM QUALIFICATIONS:
- Associates Degree in related field or equivalent; and
- 1 3 related experience which includes some leadership; or
- Any acceptable equivalent combination of education experience and training.
