The centers current scope of work includes behavioral health health & health care financing health in all policies health systems transformation long-term services & support maternal health & child well-being population & global health and rural health. The center conducts analyzes and disseminates qualitative and quantitative research to connect decision-makers with the objective research and guidance needed to make informed decisions about health policy and programs. The center is at work locally nationally and internationally with public and private partners to improve health and well-being.

JOB DESCRIPTION

The CHW program manager is a member of the Atlanta Regional Collaborative for Health Improvement (ARCHI) team providing support to ARCHIs Community Resource Hubs a key element of ARCHIs Invert the Burden strategy. ARCHI is a collaborative of 110 partner organizations with a shared 28-year commitment to improving health in metro Atlanta. ARCHI is housed inside GHPC with seven dedicated staff guiding the work of the collaborative. ARCHI supports this commitment by facilitating systems change across a wide range of sectors and policy areas while designing and implementing prototypes to test and refine innovations in the way care is delivered and financed for those Atlantans most impacted by the historic and pervasive gaps in opportunity to achieve optimal health and well-being in the community.

This is an Atlanta-based in-office position.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Care Coordination

Build strong relationships with members and exhibit excellent customer service

Act as an advocate and liaison between the member family and community service agencies

Record all care information in various platforms

Program Management

Prepare reports and documents as needed or requested. Ensure timely documentation of program outcomes

Networking skills and aptitude for representing ARCHI with local state and national organizations

Ability to initiate new and strategic relationships to support ARCHI projects; comfortable working with residents and community leaders

Flexibility to shift priorities address ad-hoc requests and manage ambiguity

Effective time management and organizational skills (e.g. ability to prioritize and meet deadlines in a fast-paced environment)

Must be a self-starter who is able to work independently and in a team environment

Partner and Resource Development

Build relationships with external stakeholders (e.g. ARCHI Steering Committee community leaders and residents).

Recruits new partners for the Community Resource Hub and maintains existing partnerships.

Other duties as assigned.

ESSENTIAL SKIILS AND ABILITIES

Able to work effectively both as a member of a team and with a high degree of independence including good judgement and problem-solving skills.

Provide one-on-one support to members including coordinating appropriate referrals for services and sharing accurate and relevant information and resources.

Knowledge of health systems care coordination and CHW programs.

Experience managing and completing grant deliverables.

Excellent attention to detail and documentation of completed services and tasks.

High level of knowledge about community agencies in the community they will be serving.

Proven ability to create and maintain strategic partnerships with external organizations.

Experience in navigating the IRB process and data security best practices.

Program evaluation including qualitative and quantitative analytic skills.

Communicate effectively with stakeholders.

Minimum Hiring Qualifications

Masters degree and five years of related experience including two years of supervisory experience.

Preferred Hiring Qualifications

Ph.D. with two years of experience; or a combination of education and related experience.

Knowledge of collective impact practices and health and racial equity is critical. Knowledge of excel is a plus.
