Job Summary:
The Physical Therapist provides physical therapy services to patients in their homes within the Medford OR and surrounding areas. This includes patient assessment treatment planning and implementation of therapeutic interventions to improve mobility function and quality of life. The therapist is expected to manage a caseload maintain accurate documentation and collaborate with other healthcare professionals.

Location:Medford Oregon United States
Responsibilities:
- Conduct patient evaluations to assess physical condition functional limitations and rehabilitation needs.
- Develop and implement individualized treatment plans based on patient assessments and goals.
- Provide skilled physical therapy interventions including therapeutic exercise manual therapy gait training and functional activities.
- Monitor patient progress and adjust treatment plans as needed.
- Educate patients and caregivers on proper techniques for exercise mobility and self-care.
- Document patient care services accurately and completely using the EMR system (EPIC/Oasis).
- Communicate with physicians and other healthcare professionals regarding patient progress and treatment recommendations.
- Maintain a productivity standard of 30 points.
- Travel to patient homes within a service radius occasionally up to 50 miles.

Required Skills & Certifications:
- Active Physical Therapist license.
- Basic Life Support (BLS) certification.
- Minimum 1 year of skilled home health experience.
- Oasis experience.
- Valid drivers license and reliable transportation with car insurance.

- No on-call requirement.
- No floating between units.
- Block scheduling is not accommodated.
