The Village of Aspen Lake located in Windsor Ontario currently has a unique exciting opportunity for a Behaviour Supports of OntarioOutreach (BSO) Coordinator to join their BSO team providing services to both the Village as well as others within the community. This role supports the Windsor community and will support residents where they are located travel throughout the community is required.

As our BSO Coordinator you will:
Support and embrace our resident-centred social culture and be proud to deliver the highest quality of personal care to our residents. You will work as key members of our Village and neighbourhood multidisciplinary teams to understand residents individual needs and advocate for residents to actively participate in decisions about their care.
BSO Coordinators at Schlegel Villages foster exemplary relationships with residents their families and the Village team to deliver holistic care focusing on flexible living and quality of life.
To be successful as our new BSO Coordinator you will:
- Hold a valid PSW Certificate OR Social Service Worker Diploma
- Display a caring and respectful attitude toward our residents families and team members
- Understand the physical mental and emotional demands of the role
- Meet the Following Core Competencies:
- Comprehensive Behavioural Assessment
- Collaboratively Develop Tailored Approaches/Strategies Contributing to a Person-Centred Behavioural SupportPlan.
- Support Implementation of the Person-Centred Behavioural Support Plan
- Monitor and Evaluate Outcomes.
- Observe team members during resident interactions particularly during care and provide coaching and guidance to adjust care approaches in order to better meet each residents individual needs
- Focus on individualized approach - assist with minimizing personal expressions for residents living with dementia and/or other neurological disorders
- Connect with Village team members family and any other care partners to collect perceptions and personhood information to assist with understanding the resident and identify potential interventions for improving personal expressions
- Provide in-the-moment coaching and support to Internal Champions and Village team members to adjust care approaches when residents are experiencing personal expressions
- Transfer knowledge in the moment embedding best practices and BSO principles. Build capacity to shift culture and thoughts around behaviours and to improve overall awareness of dementia.
- Promote the use of person-centred language
- Aide in the transition of residents transferring from community retirement and hospital to the Village
- Support outreach services throughout the Windsor community reliable vehicle is required
Have the ability to work independently prioritize resident needs when managing schedules and take initiative in supporting care strategies and team collaboration.
This is a part time hybrid position supporting the objectives of the role which requires availability to support onsite throughout Windsor in order to accommodate resident needs. Evening and weekends may be required.
