Job Description:
- Examines patients with impairment functional limitation and disability or other health-related conditions to determine a diagnosis prognosis and intervention.
- Performs and documents initial exams evaluates data to clarify and substantiate patient complaints problems and functional limitations to determine a diagnosis prior to intervention.
- Plans prepares or carries out individually designed programs of physical treatment to maintain improve or restore physical functioning alleviate pain or prevent physical dysfunction in patients.
- Records prognosis treatment response and progress in patients chart. Identifies and documents goals anticipated progress and plans for reevaluation.
- Evaluates effects of treatment at various stages and adjust treatments to achieve maximum benefit.
- Tests and measures patients strength motor development and function sensory perception functional capacity or respiratory or circulatory efficiency and record data. Instructs and educates patients on a plan of care to be continued on their own.
- Alleviates impairment and functional limitation by designing implementing and modifying therapeutic interventions.
Requirements
Minimum Qualifications:
- Diploma/transcripts to support graduation with Doctor of Physical Therapy (DPT) degree from an accredited program.
- Current valid unrestricted license as a PT issued by the Arizona State Board of Physical Therapy.
- Current completed PSV from the Arizona State Board of Physical Therapy.
- Current completed fit testing for N95 particulate respirators.
- Minimum one year of past work experience in this position.
Required Skills:
Conduct patient evaluations histories and physical exams for new and follow-up cardiology patients Manage and monitor patients with cardiovascular conditions such as hypertension coronary artery disease atrial fibrillation heart failure and hyperlipidemia Order and interpret diagnostic tests including EKGs Holter monitors echocardiograms and stress tests Assist in care coordination for procedures or specialty referrals Provide inpatient cardiology consults and follow-ups (if applicable) Participate in hospital rounds discharge planning and transitions of care Educate patients and families on lifestyle changes medications and disease management Collaborate closely with supervising cardiologists primary care providers and support staff
Required Education:
Current and unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA) in Hawaii or eligibility to obtain one National certification (AANP ANCC or NCCPA) BLS and ACLS certification required 12 years cardiology or internal medicine experience preferred; strong new graduates with relevant rotations or RN cardiology background considered Excellent interpersonal and communication skills Comfortable working independently with physician support EMR experience
