Job Summary:
The Medical Social Worker provides psychosocial evaluations formulates treatment plans and assists participants in resolving problems and meeting their biopsychosocial needs within an adult day care/outpatient community setting. This role involves making referrals to community resources collaborating with other disciplines participating in meetings completing documentation and assisting in community outreach efforts.

Location:Napa California United States
Responsibilities:
- Conduct psychosocial evaluations.
- Formulate treatment plans and goals.
- Assist participants in resolving problems and meeting their biopsychosocial needs.
- Make referrals to appropriate community resources.
- Confer with other disciplines.
- Participate in internal meetings.
- Complete documentation in a timely manner.
- Assist in community outreach efforts.
- Data entry into electronic medical database systems.
- Documents patient care and related duties appropriately.

Required Skills & Certifications:
- BLS certification
- Minimum 1 year of prior social work/clinical experience in a healthcare setting
- Charge experience

Preferred Skills & Certifications:
- Experience in adult day care case management home healthcare LTC/nursing homes or rehab settings
- Experience in adult day care case management home healthcare LTC/nursing homes or rehab settings
Special Considerations:
- Candidate must have reliable transportation and no additional scheduling restrictions.
- All travel pairs will be required to have their own transportation.
- Facility may call off traveler up to 1 shift per pay period (every 2 weeks).

Scheduling:
- 5x8hr Days (Monday-Friday)
