SUMMARY:
The LPN Transitional Care Management Program is responsible for implementing and maintaining the program for patients transitioning to home care for all primary care departments.
PREFERRED QUALIFICATIONS:
Formal training which will probably be indicated by a graduation certificate from an accredited LPN school of nursing; basic life support and CPR certifications; and current Kentucky state licensure. Minimum two years clinical experience with emphasis in primary care medicine preferred.
- Knowledge of clinical standards quality assurance and reporting. Previous experience with care management or other applicable programs preferred.
- Proficient in Word Excel and other computer programs.
- Demonstrate sound judgment and critical thinking skills.
- Ability to work independently and collaborate with other team members.
- Excellent communication skills both written and verbal. Ability to listen well and work closely with patients staff and providers.
- Demonstrated self-initiative and willingness to take on new responsibilities.
PHYSICAL GUIDELINES:
Physical guidelines include the ability to move traverse position self remain in a stationary position and negotiate steps for up to eight hours per day; lift move and transport patients and materials; manual dexterity and motor coordination; articulate speech; read and understand medical information; visual acuity.
