Job Summary
Wootton Vale Healthy Living Centre are looking for an enthusiastic and skilled practice nurse to join our friendly and forwardthinking primary care team. This position can be offered on a full or part-time basis with flexible options available.
If youre passionate about providing highquality patient care enjoy working collaboratively and have strong experience in longterm condition (LTC) management wed love to hear from you.
As a key member of our clinical team youll deliver a wide range of nursing services and support patients in managing chronic health conditions. We are particularly interested in someone who can take a proactive holistic approach to patient care.
Your responsibilities will include:
- Management and review of patients with longterm conditions (e.g. diabetes asthma COPD hypertension)
- Supporting longterm condition clinics and QOF-related work
- Providing high-quality general practice nursing services such as immunisations wound care cervical screening health checks and lifestyle advice
- Working autonomously while contributing to the wider multidisciplinary team
- Supporting practice initiatives to improve patient outcomes and access
About the Candidate
- Current NMC registration
- An experienced practice nurse working in a GP or primary care setting
- Skilled in longterm condition management with confidence running LTC clinics
- Knowledgeable in relevant clinical guidelines and best practice
- A great communicator with a compassionate patient-centred approach
- Organised proactive and able to work both independently and within a team
Desirable:
- Independent Nurse Prescriber (V300) qualification
- Competence in managing and prescribing for LTCs
- Experience using GP clinical systems such as EMIS/SystmOne (if applicable)

We are a friendly surgery with a list size of over 18000 patients over two sites Wootton Vale (MK43 9JJ) and branch site Shortstown Surgery (MK42 0GS). We have good appointment access which translates to happy patients and a strong teamwork ethos that includes an established clinical pharmacy team GPs nurses physician associates clinical liaison and admin teams. We are also a tier-2 sponsoring practice and would support any applicants requiring a tier 2 visa.
