At Senior Solutions (Area Agency on Aging in Southeastern Vermont) we are seeking aCase Manager Outreach Specialistto work with older adults and/or people with disabilities inSpringfieldVT and the surrounding areas. This positionacts as an agent for the client with the goal of assessing their needs and implementing a planning process to empower the client (and caregiver where applicable) to use program services and available resources appropriate to meet those needs and create future sustainability for independent living. The Case Manager Outreach Specialist will work with clients and their caregivers to develop individualized long-range planning options and complete benefits applications. There is special emphasis on assisting those most at risk of losing independence.
Qualifications:
Associate or bachelors degree preferably with a in human service orientation and at least 3 years of social service or related experience.
A combination of relevant education and experience may be substituted for a degree.

Must have reliable transportation and a valid drivers license.
Responsibilities include:
  • Recruit and enroll eligible older adults in Choices for Care Medicaid long-term care 3SquaresVT fuel assistance Medicaid Medicare savings (LIS) and other public benefits programs.
  • Assist older adults with eligibility requirements (filling out forms gathering information etc.) tracking enrollment challenges and successes and serving as a liaison between seniors and state program staff.
  • Complete Choices for Care Medicaid intake Independent Living Assessments (ILA) on clients referred from state Long-term Care Clinical Coordinators.
  • Create monitor and update a person-centered care plan to support Older Americans Act clients choices related to their needs and wants that enhance their independence.
  • Work in collaboration with other service providers to carry out the person-centered care plan.
  • Participate in case consultation with multi-agency teams on behalf of clients when appropriate.
  • Adhere to policies procedures and standards of performance for case management as established by the State of Vermont.
  • Frequent travel to client locations. Reliable transportation and valid drivers license required. Minimum car insurance required by state law.
  • Ability to occasionally lift and/or carry objects weighing up to 25 pounds. Must possess the physical capabilities to perform the following: reaching handling talking hearing seeing standing walking stooping and bending.
  • Work environment involves minimal exposure to physical risks. Visitation to clients may come with some risks (unsanitary homes hoarding smoke).
  • Recordkeeping work is normally performed in a typical interior/office work environment
