The Integrated Behavioral Health Consultant (IBHC) works under the supervision of the Director of Behavioral Health and performs all functions duties and responsibilities within the primary care department as a member of the care team. The IBHC provides quality clinical mental health consultation to patients based on the needs of the patient. Additionally psychosocial care will be given based on factors of the Social Determinants of Health (SDOH). This may include the following: food insecurities income levels homelessness addictions specific evaluations based on disabilities. All SDOH may be included in the integrative health plan of care.
Key Accountabilities:
- Provides clinical consultation to primary care providers (PCP) and staff for elders adults adolescents children and families regarding mental and/or behavioral health related issues that relate to patients medical condition(s).
- Documents all patient-provider interactions following health care standards.
- Participates as an active member of an integrated patient centered team.
- Responsible for crisis triage and intervention.
- Supports the organization as needed.

- Ability to actively participate as a member of the interdisciplinary primary care and mental health care team.
- Ability to provide written or oral reports and evaluations concerning program activities accomplishments goals and objectives.
- Knowledge of existing social and behavioral health resources available for patient care.
- Open to change and to lead change within the organization.
- Capable of making independent professional judgements and recommendations for medical social and psychosocial intervention.
Qualifications :
- Current valid and unrestricted license of independent practice in the State of Arizona- LCSW LPC or LMFT are required.
- A masters degree in social work or related discipline from an accredited program is required.
- Three years of experience as a behavioral health provider is required.
- Bilingual (Spanish/English) preferred.
- Experience in NextGen Electronic Health Record preferred.
- Experience working with a diverse population in a community health setting is preferred.
- Proficient with computer-based applications such as MS Office Adobe and electronic health records.
Standard Employment Requirements
- Must possess a valid and unrestricted Arizona drivers license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona drivers license during employment. Must be insurable under NATIVE HEALTHs liability auto policy.
- Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
- Must pass substance abuse testing upon employment and submit to random testing during employment.
- Must provide an updated immunization (IZ) record at the time of employment.
- Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
- Must obtain BLS certification within (90) calendar days of employment and maintain certification during employment.
