Job Summary:

The Registered Nurse (RN) will provide skilled nursing care case management and hospice care to individuals. Responsibilities include patient assessments medication administration treatment planning and interdisciplinary team collaboration. Strong communication and collaboration skills are essential.

Responsibilities:

- Provide skilled nursing care.
- Conduct patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Float between units as needed.
Required Skills & Certifications:

- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum one year of hospital home health or hospice care experience (1YR/1ST TIMERS OK)
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of working
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
Preferred Skills & Certifications:

- Specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room.
Special Considerations:

- Must be comfortable working in ICU (1:2 ratio) and Step-Down (1:3 ratio) environments.
- Weekend shifts are required (6 weekend shifts per schedule period).
- Floating between units is required.
- 48-hour shifts are not approved.
