Job Summary

The South West Aboriginal Medical Service (SWAMS) is a non-Government Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West of Western Australia. SWAMS delivers a wide range of community programs and has a strong growth strategy through partnership opportunities and future community development.

A Registered Nurse Client Coordinator within the Community Services team at SWAMS ensures the ongoing provision of the highest standards of care to clients of the service through the provision of community services and health education. SWAMS staff strive to implement best practice Community Services care and to coordinate multidisciplinary care planning internal and external referral including referral for specialist care and allied health.

This role is not your typical clinical RN role and is NOT based at the SWAMS clinic. You will be working within an Allied Health team that provides support to the Aged Care Services at SWAMS with a primary focus on Aged Care Management (Support at Home). This role will see you regularly out in community meeting with our clients. This role is open to applicants who are interested in either full time for part time hours. This includes:

  • Provide relationship-based care management to older clients and their carers/families using a person-centered rights-based and culturally safe approach.
  • Undertake comprehensive assessments and develop individualised care plans that incorporate client goals cultural needs risk factors wellness and reablement Assistive Technology (AT) and Home Modifications where applicable.
  • Deliver ongoing care management activities including monthly monitoring/contacts and care plan review at least annually or when needs change.
  • Coordinate and navigate services across support streams; liaise with providers community agencies and SWAMS teams to ensure timely tailored supports.
  • Escalate or integrate clinical interventions as needed applying advanced RN skills when client needs exceed general care management scope.
  • Maintain accurate documentation budgets and records for aged care participants in accordance with program and organisational requirements.

About you:

You will be a passionate Registered Nurse who is looking to work within a NFP that directly supports the local Aboriginal families and community. You will also have the following:

  • Registered Nurse with at least 3 years post-graduation experience in nursing.
  • Current registration with AHPRA
  • Demonstrated ability to communicate effectively and sensitively with Aboriginal people
  • A demonstrated understanding of the unique issues effecting and impacting upon the health of Aboriginal people
  • Demonstrated organisational and time management skills along with an ability to adapt to changing needs

It would also be beneficial but not essential to have the following experience:

Experience in Community or Aboriginal Health
