As an Advanced Practice Provider youll play an integral role in our inpatient and ambulatory care teams by providing high-quality cost-effective care to patients.
We are seeking a Nurse Practitioner or Physician Assistant to:
- Provide care primarily to patients with diabetes thyroid disorders osteoporosis and/or general endocrine disorder.
- Work collaboratively within a multidisciplinary health team to provide comprehensive care to both well and ill patients in an outpatient (85%) and inpatient setting (15%).
- Participate in the daily functions of the Diabetes Management Service.
- Conduct comprehensive or episodic health history and physical assessment of patients/families with acute chronic and/or potential health problems.

Education:
Minimum
- If Nurse Practitioner: Masters or Doctorates degree from accredited Nursing Program.
- If Physician Assistant: Bachelors or Masters degree from accredited PA Program.
Licenses and Certifications:
Minimum State of Wisconsin NP or PA licensure
If Nurse Practitioner*:
- Active RN and APRN certifications and licenses including prescriptive authority in state(s) of practice AND
- Active board certification as a NP in area of clinical practice. These may include the following: Adult; Adult/Gerontology Acute Care; Adult/Gerontology Primary Care; Emergency; Family.
If Physician Assistant*:
- Active PA license including prescriptive authority in state(s) of practice AND
- Active PA board certification
All Nurse Practitioner and Physician Assistant applicants must have*:
- Active DEA license or ability to apply for such license prior to or upon hire.
- Active license or eligibility to obtain active licensure in state of practice. This may include licensing in Wisconsin and other state(s) of practice.
